Client Intake

Please submit a Client Intake request to be added to our Client List. You will receive email and text notifications, along with access to any current wait-list, and an expedited Online Booking experience.

Submitting an intake form through this link will only collect information about you and your pet(s). No Card-on-File will be collected and no appointment will be made. You will receive a notification from our automated system when your client request is approved and you may reply within our system with any further questions or requests.

If you would like to request an appointment at this time, you may continue to the Online Booking link and submit the Client Intake, Service Agreement, and Card-on-File forms in a single process. Please be aware though that if you do not complete the booking request your information will not be saved. Continue to Online Booking instead.




Digital Agreements

Please take a moment to read our Digital Agreements and save for your records. The Service Agreement is required for every client upon intake. Special Release of Liability Forms are only required if a client elects an Excluded Service for their pet and will be sent to individually clients as needed.
